IDD Residential Supervisor

GENERAL DESCRIPTION

Work is performed under the general supervision of the Program Administrator of Provider Services with moderate latitude for use of initiative and independent judgment. Responsibilities include: Ensure that each home has food items in accordance with the home’s menu per the dietician’s recommendations and in accordance with ICF guidelines, completing home visits to ensure that the group homes are clean, ensuring that fire/safety drills are completed, assisting with safely transporting individuals, ensuring that maintenance request are completed in response to maintenance issues in the group homes, ensure that the group homes have adequate household supplies, ensure that training goals are completed for the individuals living in the group homes, support the individuals served in achieving their desired outcome, assist the individuals served with training goals, assist the individuals served with participation in community inclusive activities, and assist the individuals served with utilizing daily living skills / achieving personal outcomes and monitoring the individuals served to ensure safety. Complete documentation of all services and other related supports prior to the end of shift. Must serve as the primary contact for on-call scheduling, overtime (when necessary), and coverage for all shifts. Communicate with Care Coordinators and relevant team members, in accordance with the needs of the individuals served and the needs of the group homes. All activities will support the mission of LifePath Systems by putting the individuals served and their families at the center of planning and service delivery with consideration of their needs and preferences. Comply and adhere to HCS principles, ICF regulations / standards as well as LifePath Systems rules, policies and procedures.

EDUCATION, TRAINING, & EXPERIENCE

  • Preferred: Graduation from an accredited four (4) year college or university with a degree in a Human Services related field.
  • Must have a minimum of 2 years’ experience in IDD services.
  • Must have excellent organizational and communication skills.
  • Must have basic competency in the use of Word, Outlook, Excel, and the ability to achieve competency in the use of an electronic health record.
  • Prefer someone with experience working at a Community Center.

ESSENTIAL TASKS & STANDARDS

  • Function as a contributing and effective team member. This includes sharing appropriate resources, techniques, and strategies with other team members.
  • Follow all agency policies, procedures and guidelines on the use of agency property and resources.
  • To document the percentage of time spent on core activities of direct services support.
  • Complete and turn in all HCS/ICF documentation prior to the end of each shift.
  • Ensure physical /safety measures are met to ensure the safety and well-being of each individual.
  • Treat individuals with respect and dignity.

KNOWLEDGE, SKILLS, & ABILITIES

  • Working knowledge of Home and Community Based Services and TxHmL standards and rules.
  • Ability to communicate clearly, both orally and in writing, in English.
  • Ability to communicate in a timely manner with all related parties.
  • Ability to organize and prioritize a variety of assignments while managing time effectively.
  • Ability to form and maintain effective relationships with individuals receiving services, families, co-workers, other Center personnel, contractors and the public when representing LifePath Systems.
  • Ability to utilize an electronic health record, electronic payroll system and other technology applications as necessary

CERTIFICATES, LICENSES, & REGISTRATIONS

  • Must have valid Texas Driver License and a good driving record.
  • Must be able to pass a criminal background check.
  • Must be able to complete all LifePath Systems training.
  • Must be able to provide at least 3 professional or educational references. 

BENEFITS

Salary Range: $18 - $20 per hour           

Shift Hours: Monday through Friday (This position will manage all Direct Support Professional staff across two HCS group homes and two ICF group homes in Plano, TX. It is a 40 hours/week position but will involve creating and flexing your schedule to fit departmental needs).
