GENERAL DESCRIPTION

Work is performed under the general supervision of the Provider Services Program Administrator with broad latitude for use of initiative and independent judgment. As Care Coordinator, coordination responsibilities include accessing, evaluating, and identifying the needs of individuals served within the community and residents of the group homes, monitoring the service plans of individuals on caseload to ensure that all services are provided to the satisfaction of the individual receiving services. Coordination responsibilities also provide program coordination, and development/monitoring of treatment plans. Working within a team (Care Coordinators and the QIDP) to aid with the group home residents as needed.  There is a requirement to participate in the after hour on-call rotation schedule. Care Coordinator is required to participate in all Provider Services Program team. Care Coordinator will ensure that training goals are completed for the individuals living in the group homes, support the individuals served in achieving their desired outcome, assist the individuals served with training goals, assist the individuals served with participation in community inclusive activities, and assist the individuals served with utilizing daily living skills / achieving personal outcomes and monitoring the individuals served to ensure safety. Complete documentation of all services and other related supports prior to the end of shift. Communicate with appropriate team members in accordance with the needs of the individuals served and the needs of the group homes. All activities will support the mission of LifePath Systems by putting the individuals served and their families at the center of planning and service delivery with consideration of their needs and preferences. Comply and adhere to HCS & TXHML principles, ICF regulations / standards as well as LifePath Systems rules, policies, and procedures.

EDUCATION, TRAINING, & EXPERIENCE

  • Graduation from an accredited four (4) year college or university with a degree in a Human Services related field.
  • Must have a minimum of 2 years’ experience in IDD services.
  • Must have excellent organizational and communication skills.
  • Must have basic competency in the use of Word, Outlook, Excel, and the ability to achieve competency in the use of an electronic health record.
  • Prefer someone with experience working at a Community Center.

ESSENTIAL TASKS & STANDARDS

  • Monitoring Programing to meet client needs; Provide services as outlined in the treatment plan.
  • Oversee Financial Accounts.
  • Submits required paperwork in a timely manner.
  • Assist with providing services within the ICF and HCS group home setting as needed.
  • Monitor the physical and safety condition of residential and SHL individuals living independently. To locate and coordinate services and supports to prevent and/or manage crisis situations as identified by individuals
  • Use or direct service time. To document the percentage of time spent in core activities of service coordination.
  • Number of strategies/trainings implemented to assist individuals with meeting their needs.
  • Demonstrate and maintain professional relationships and behavior with the individuals served, family members, stake holders, co-workers, supervisors, and the public. Function as a contributing and effective team member. Perform and function as a contribution member of the IDD Team. This includes sharing of appropriate resources, techniques, and strategies with other members of the team
  • Understands and implements Trauma Informed Care principles.
  • Follow all agency procedures and guidelines on use of agency property and resources.

KNOWLEDGE, SKILLS, & ABILITIES

  • Working knowledge of Home and Community Based Services and TxHmL standards and rules.
  • Ability to communicate clearly, both orally and in writing, in English.
  • Ability to communicate in a timely manner with all related parties.
  • Ability to organize and prioritize a variety of assignments while managing time effectively.
  • Ability to form and maintain effective relationships with individuals receiving services, families, co-workers, other Center personnel, contractors and the public when representing Lifepath Systems.
  • Ability to utilize an electronic health record, electronic payroll system and other technology applications, as necessary.

CERTIFICATES, LICENSES, & REGISTRATIONS

  • Must have valid Texas Driver License and a good driving record.
  • Must be able to pass a criminal background check.
  • Must be able to complete all LifePath Systems training.
  • Must be able to provide at least 3 professional or educational references. 

BENEFITS

Salary Range: $40,000 - $45,000 per year          

Shift Hours: 8am - 5pm
