GENERAL DESCRIPTION
Position exemplifies service excellence in providing home and community based routine case management, skills training, medication training and support, and crisis interventions utilizing a trauma informed, person driven care model. Maximizes resources to enhance Lifepath Systems’ operations and supports individual and family strengths and needs. Demonstrates integrity in role by ensuring state standards and Lifepath Systems policies, procedures, and values are met. Works independently and collaboratively as a team member and community builder. Continuously improves in measurable ways on how to support individuals in services. Is responsible for initiating and collaboratively completing uniform assessments then organizing and documenting the plan for services to meet the individual’s identified needs. Must be effective at troubleshooting and organizing support, interventions, and services to achieve outcomes. Expectations include timely and thorough documentation, time management and organizational skills, effective scheduling, ability to efficiently manage clinic flow, adhere to crisis procedures and be available to assist with crisis and other coverage areas as needed. Must have ability to effectively communicate with supervisory staff and peers when necessary.

BENEFITS
Salary Range: $40,000 - $48,000 per year
Shift Hours: Hours vary depending on the needs of individuals served.
